WELCOME TO OUR ORTHODONTIC PRACTICE
We hope you will decide to use our services. Here are some of the things which will take place during your first thee visits.

THE FIRST APPOINTMENT is primarily to determine if there is a need for orthodontic treatment and, if so, when is the best time to begin. We will explain, in general, what treatment may be required and approximate estimates of the time and costs involved. We say "in general" because we will not know exactly what will be required until a complete review of Diagnostic Records is made.

THE NEXT APPOINTMENT will be to make the Diagnostic Records. These records may include dental impressions with which to make plaster models of the teeth, x-ray pictures of the teeth, head and jaw joint (TMJ), and photographs of the teeth and face. A Records Fee of $230 is due when records are taken. This fee will be applied toward the total fee for the orthodontic services. More about fees later.

THE THIRD VISIT is a Consultation in which the diagnosis will be discussed in detail, the exact fee established and the time of treatment estimated. In the case of a child, the patient and both parents should come to the Consultation. For adults, anyone with an interest in the patient's orthodontic care should be present. Financial arrangements will also be made at this visit.

A WORD ABOUT FEES
The range of fees for complete orthodontic treatment (braces) is usually between $4400 and $5400. Fees for interceptive care for very young patients or functional appliances for the growing patient will generally range from $1200 to $2400. These fees are virtually all-inclusive except for extractions, if required. Every effort will be made to set up an Initial Payment and Monthly Payments tailored to your needs. These arrangements are made with our Practice Administrator. The Records Fee is considered to be a part of the Initial Payment.

INSURANCE
If you are fortunate enough to have orthodontic coverage, we will be happy to complete the Doctor's portion of the claim form. Please understand, however, that you are responsible for all service rendered and that your insurance company is responsible to you. We feel it is important to preserve the traditional, personal, two-way patient-doctor relations which has served us all so well over the years.

THANK YOU FOR CONSIDERING US
We look forward to having you as a patient in our practice.
