Appointments
Your scheduled appointment time has been reserved specifically for you. We request 
24-hours notice if you need to cancel your appointment. We are aware that unforeseen events sometimes require missing an appointment. After missing your second appointment without notifying us 24 hours in advance, you are subject to being charged an additional fee.

Financial Info
It is our mission to provide you with affordable, high-quality treatment that fits your budget! Your treatment plan will include a breakdown of all applicable fees, and we will inform you of all costs before treatment is administered. We offer a variety of payment options to meet your needs. For your convenience, we accept Visa and MasterCard. We also offer no-interest financing plans through CareCredit to help make your treatment more affordable. Courtesies are available for multiple family members undergoing treatment simultaneously, as well as for patients who pay in full at the onset of treatment. We thoroughly explain your options, so that we can accommodate your needs.
Dental Insurance
We will submit all insurance claims for you. We will fully attempt to help you receive full insurance benefits; however, you are personally responsible for your account, and we encourage you to contact us if your policy has not paid within 30 days.
To help us with your insurance filing, please complete the insurance questions on our questionnaire.
Orthodontic Insurance
If you are fortunate enough to have orthodontic coverage, we will be happy to complete the Doctor's portion of the claim form. Please understand, however, that you are responsible for all service rendered and that your insurance company is responsible to you. We feel it is important to preserve the traditional, personal, two-way patient-doctor relations which has served us all so well over the years.
